BACKGROUND AND PURPOSE OF THE ASSIGNMENT

Postpartum hemorrhage (PPH) occurs in approximately 5% of all live births and, despite concentrated efforts, remains a leading cause of maternal morbidity and mortality. Because most PPH‐related deaths are preventable through the implementation of effective interventions, the recent shift from home births to facility births across low‐ and middle‐income countries (LMIC) raises new opportunities for saving women's lives. Unfortunately, inconsistent and/or delayed use of effective interventions for prevention and treatment of PPH, in addition to other systemic problems in health services (e.g., lack of blood banks, inadequate staffing), has led to continued unacceptable rates of haemorrhage‐related maternal deaths. Care bundles have been associated with improved patient outcomes when adherence is high. The concept of care bundles is similar to that of packages and checklists, which have been used by healthcare providers for decades with a similar goal of standardizing and expediting care. Care bundles may include behaviors, such as the widely used “ABCs” designed to help practitioners remember the sequence for resuscitation, or a number of interventions packaged together, such as the “Active Management of the Third Stage of Labor” (AMTSL) package used to prevent PPH.

In 2012, WHO published its “Recommendations for the Prevention and Treatment of Postpartum Haemorrhage” to provide evidence‐informed clinical care recommendations for haemorrhage due to uterine atony. However, adherence to these recommendations remains a challenge. The bundle approach has been proposed as a potential solution to suboptimal adherence to PPH guidelines. Healthcare bundles have been proposed for maternal conditions including placenta previa, elective induction, labor augmentation, vacuum delivery, maternal sepsis, and obstetric anal sphincter injury, but evidence of their success or failure is lacking.

Together with our partners and local MNH stakeholders including the Professional bodies like Kenya Obstetrics & Gynaecological Society (KOGS) – KMET developed and iterated a PPH EMC kit (Bundle). The bundle approach has made Healthcare workers work in a much more systematic fashion as compared to previous efforts which were much more disorganized. KMET through her social enterprise entity - Center for Maternal Health Innovations (CMHI-Ltd), is seeking to develop, commercialize and accelerate uptake of maternal health innovations towards positive social impact. KMET therefore seeks to engage a consultant to provide technical assistance in the development of a business and commercialization plan for PPH intervention products in Kenya.

 

CONSULTANCY OBJECTIVES:

The objective for developing the Business Plan for the CMHI is to provide guidance on the operations of the system/structure.

Specifically, the Business Plan will:

  • Define the CMHI capacity in terms of capacity building activities and other technical assistance (TA)
  • Define the organizational management structure and skill sets available and identify any skill gaps
  • Define the running cost of the CMHI, broken down to the cost per product/services provided, including services like trainings and TA
  • Define the comparative advantage of the CMHI to attract customers from countries in the East Africa Region
  • Define the market and the potential customers
  • Describe the business model, including the Commercialization strategy
  • Work out the appropriate costing, including a portion for core funding, for each product provided
  • Project a realistic income from marketing the services over the next five years
  • Project the time to reach the break-even status from the present funding and donor support (funds) required till then
  • Provide organizational management structural modification and new skill set (if required) to meet the requirements of the business plan

SCOPE OF WORK

Based on objectives the consultant(s) will be required to provide an analysis of the present scenario and develop a detailed business, costing and marketing plan that is expected to provide overall direction to the CMHI on operations making it sustainable, efficient and growth oriented in the long run. The plan should be time bound and show clear achievable targets within defined timelines.

The Business Plan to have the following sections at the minimum:

  1. Executive summary
  2. Analysis of the services offered
  3. Analysis of the organizational status, skill sets, running cost (broken to each Product) and gaps thereof
  4. Analysis of the market and potential customers
  5. Comparative advantage
  6. Business Model and description of operation/s for each Product
  7. Costing for each product
  8. Marketing strategy
  9. Modified organizational management structure & new skill sets
  10. Projection of business returns over 5 years
  11. Financial/funding needs till break-even status

The plan should also have a section on general guidance and preparation of an annual budget, 5-year (Financial model) projections of the following statements:

  1. Projected sales/income statement
  2. Projected cash flow statement
  3. Projected balance sheet.

A risk assessment, financial ratios and key performance indicators such as break-even, turn-around time, profitability ratios, payback period, rate of return on investment, liquidity ratios and stock turnover in order to have a better understanding of the financial and operational performance. A realistic sale forecast including prices, sale volume, timing, etc.

 DELIVERABLES

The expected end product of this assignment will be a (1) complete Business and Marketing Plan clearly detailing tasks and responsibilities of each actor within the systems; (2) a financial and operational plan; (3) and a plan for measuring performance and impact of the system.

TIME FRAME

The above deliverable must be completed within 60 working days from the start date, with the following timeline (based on one consultant):

  1. Preparatory activities: – 5 days
    • Review all relevant project documents and materials
    • Consultation with CMHI, KMET teams on the history, Department of Reproductive Health - Ministry of Health, Procurement departments, previous buyers and other stakeholders
  2. Situation analysis: - 20 days (including travel days)
  • Desk review
  • Site visits and interviews with key informants and/or focus groups
  • Data analysis
  • de-briefing
  • During the field work, the consultant will be joined for at least a week by a CMHI staff. Outcome: situation Analysis report
  •  
  • 3. Writing the plan – 25 days (the plan should be written section-wise and each shared for comments and modification)
    • Writing and editing report
    • Incorporating comments and modification from partners
    • Finalizing business and marketing plan
    • Presentation to the CMHI and the funders on preliminary report
  • 4. Final compilation and presentation – 10 days
    • Presentation of the first draft, in a workshop mode, to a select audience
    • Recording of feedback and making the needed changes
    • Final Business and Marketing plan submitted to CMHI
Qualifications

We are looking for a firm/individual with the following Experience;

  • Working with regional or national medical/scientific institutions
  • Analysing sets of services provided by health marketing firm/ establishments
  • Analysing market preferably of health services/health products
  • Providing costs analysis, projection of income and returns
  • Analysing skill sets and organizational structures required of different groups of functionaries in a
  • multi-dimensional set-up preferably in medical/health/educational set-up
  • Analysing national and regional strategies, policies and priorities for health sector in East Africa Region?
  • Developing marketing strategy preferably for health services or health product
  • Writing business plan preferably for health services or health products
  • Team must have at least 2 people with business and health background
